Instructions:

1. Click this link:

2. Sign in to your Google Account. If you are already signed in, skip to step 3.

3. Click “Make a Copy” button.

4. Now your Sheet is created and saved to your Google Drive. Go up and select “Tools”, then “Manage Form”, then “Edit Form.”

5. In a new browser window, you will now have a copy of your own form in Google Drive.

6. You can edit the form to better suit your needs, or you can use hit “Send” in the top right corner, get the link, copy and paste the link into your browser so that you can use the form as is to create your next brilliant outline!

All of your responses, once submitted, will be available in the Google Sheet you created in step 3, or under the “Responses” tab in the Google form.
